• Last Name
• Phone Number
• Email Address
• Username
• Password
• Billing Address
• City
• State/Province/Region
• ZIP/Postal Code
• Country
• Reason for Cancellation
• Last 4 Digits of Card
• Date of Last Charge
• Amount of Last Charge

Follow these steps:

1. Use your internet browser to visit https://www.walmart.com/help
2. Hit the link that says the chat bubble in the bottom right corner
3. Hit the link that says the text box and type “agent”
4. So that they can locate your account, provide the information they ask for
5. Choose “Start Chat”

7. Inform them that you are contacting them to cancel
8. Ask for documentation that you contacted them
9. As usual, just in case, keep the proof that you’ve cancelled handy.

3. You’ll need to login with this info: email and password
4. Go down to find “Manage MY Membership”
