• First Name
• Last Name
• Phone Number
• Email Address
• Username
• Password
• Billing Address
• City
• State/Province/Region
• ZIP/Postal Code
• Country
• Reason for Cancellation
• Last 4 Digits of Card
• Date of Last Charge
• Amount of Last Charge

Follow these steps:

1. Contact them by calling 1-888-744-00149
2. Inform them that you are contacting them to cancel
3. Ask for documentation that you contacted them
4. So that they can locate your account, provide the information they ask for
5. As usual, just in case, keep the proof that you’ve cancelled handy.

1. Type up an email and include your account information.
2. The subject line of your email MUST READ “Terminate My Account”
3. Inform them that you are contacting them to cancel
